Mandatory Departure/Vacant Unit Form

These procedures are critical to the safety of your unit, other units in your building and are mandatory for all units.
Explanatory Comment: As of May 2019, this form replaces the
previous Departure Notice. The previous form focused on
“persons”, rather than the current focus on “units”. Accordingly,
in those rare instances where an owner(s) owns multiple units,
separate, individual "Departure/Vacant Unit Forms" must be submitted
FOR EACH UNIT to which the vacancy conditions apply.
1. This form (and related procedures) is REQUIRED for any unit that will be vacant for more than 72 hours (three days).
2. If the unit will be vacant between December 1st and March 15th, please complete the appropriate additional sections below.
3. Be sure to complete the process by clicking on "submit". Your form will then be immediately sent.
